The Children’s Community Nursing Service (CCNS) is made up of different teams of specialist nurses who support babies, children and young people across Leeds. Our teams help children stay where they feel most comfortable, reduce the need for hospital visits, and make sure families feel confident and supported.
Many of the children we work with are supported by more than one of our teams, and we work closely together to provide joined up care that meets each child’s individual needs.
