Person centred questions when discussing high risk foods

Talking about choking risk can feel sensitive, especially when food is linked to comfort, identity and daily enjoyment. This section provides a range of person‑centred questions you may find helpful when discussing high‑risk foods with someone.

You do not need to ask every question. Instead, have a look through the examples and choose the questions that feel right for the person, the situation, and the relationship you have with them. The aim is to support a gentle, respectful conversation that balances safety with autonomy.

These questions help you understand what matters most to the person, explore their experiences, and work together to find safe, meaningful food choices.

Person‑centred questions

Understanding food preferences

  • “Which foods do you enjoy the most?”
  • “Are there foods that feel important or meaningful to you?”
  • “Are there any foods you wouldn’t want to give up unless absolutely necessary?”

Exploring current experiences

  • “Have you noticed any foods that have felt harder to manage lately?”
  • “Has anything made you cough or feel uncomfortable recently when eating?”
  • “Has anyone mentioned concerns, or have you spotted anything yourself?”

Understanding feelings about safety

  • “How do you feel about trying to avoid foods that may increase choking risk?”
  • “Do you feel worried about choking again, or fairly confident at the moment?”
  • “Would you like help finding safer alternatives to foods you enjoy?”

Supporting shared decision‑making

  • “What changes would feel acceptable for you right now?”
  • “Would you prefer to adapt certain foods rather than avoid them completely?”
  • “Would it help to look at the high‑risk foods list together and decide which ones matter most to you?”
  • “How can we help make meals feel safe but also enjoyable for you?”

Respecting autonomy

  • “It’s your choice, what feels right for you when we think about risks and benefits?”
  • “Would you like reminders about safer options, or prefer to decide at the time?”
  • “Is there anything we can do differently to make eating easier or more comfortable?”

Personalising support

  • “Would smaller pieces or extra sauces help you manage this food more easily?”
  • “Are there alternatives you like that feel easier to manage?”
  • “Would you like us to prepare certain foods in a different way to make them safer?”

These questions help open honest, respectful conversations that balance safety with personal choice. By asking what matters to the person and working together, you can create a mealtime plan that protects wellbeing while still honouring preferences, routines and identity.

Your insight as a carer, what you observe, notice and gently explore, plays a vital role in helping the person stay comfortable, confident and safe. Person‑centred discussions empower people to make informed choices and keep mealtimes positive and meaningful.
