Welcome to the Leeds Community Diabetes Service.

The diabetes service is a team of clinical specialist nurses, dietitians, diabetes facilitators, pharmacists and GPs with a specialist interest in diabetes. We work closely with the diabetes team at Leeds Teaching Hospitals, GP practices and Neighbourhood Teams to support people living with Type 2 Diabetes who need more specialist care.

What we can offer?

We are a short-term specialist service for patients who are currently taking the maximum dose of oral medication to control their Type 2 diabetes. Usually this is patients with a HbA1c (a blood test to show your average blood glucose levels over the last 2-3 months) of over 58 who require specialist intervention from diabetes specialist nurses, dietitians and GPs with a specialist interest in diabetes. This might mean that we look at insulin as the next step.

We can deliver education in a group, by video, in a clinic setting or by telephone with a specialist clinician depending on your individual requirements.

Who is it for?

People with Type 2 Diabetes that need to start insulin treatment or support managing their diabetes with insulin. This might be resolving hypoglycaemia or support managing high blood glucose levels. We encourage self-management of diabetes and can help you to adjust your own insulin doses appropriately as well as looking at how changes to what you eat might help to keep your blood glucose levels more stable across the day.
