Neighbourhood clinics

The Neighbourhood Clinics are a nurse-led service which provides nursing care to patients with wounds, catheters, nephrostomies and many other care needs in a clinic setting.

What we offer

We provide comprehensive nursing assessment, management, and education for individuals with complex care needs, including, wound care, leg compression therapy, Doppler assessments, catheter and line management, stoma care and a wide range of other specialized services.

Who do we work with?

The Clinic team work closely with other community healthcare staff including, tissue viability nurses, district nurses, GPs, dieticians, podiatrists, occupational therapists and physiotherapists, as well as Leeds Community equipment

Where are the clinics?

We host clinics across the city in health centres, community centres and GP practices. The service provides assessment, treatment and advice for patients with chronic wounds, stomas, catheters, nephrostomies and many more conditions.

The service can also provide advice and support to carers and family members.

Your appointment

If you’re unable to attend your scheduled appointment or need to make a change, please let us know as soon as possible. Early notice helps us offer that time to another patient in need of care.

You can contact us by phone to cancel or reschedule your appointment.

Self-referrals

Your community health hub offering convenient and compassionate nursing care close to home, including:

  • Catheter care
  • Line care
  • Wound care
  • Compression
  • Doppler assessments
  • Nephrostomy care

The clinics are a Monday to Friday service for non-urgent care if you are requiring immediate attention, please contact NHS 111 or 999.

Patients can self-refer using the online referral form. However, if professionals believe a handover would benefit and maintain the patient’s safety, a referral should be completed on their behalf.
