We are the Community Stroke Rehabilitation team and we deliver stroke specialist rehabilitation in the community for up to twelve weeks.

People who…
Have had a stroke
Are medically stable
Are over 18 years old
Have a Leeds GP
Are referred from hospital
Can be safely cared for at home
Have rehabilitation goals that can be met in 12 weeks

What we do
We offer three different pathways including Self-management, Rehabilitation and Living Well. The pathway that best suits your needs will be agreed whilst you’re in hospital.
We have a variety of ways of delivering care. These include:
Telehealth (groups, phone, video calls and apps)
Clinic appointments
Home visits
Next steps
Once your episode of care is complete, we will write to you, your GP and others involved in your ongoing care.
If you require further support, we can refer or signpost you to other services that can help.
