Clinical Triage Hub
The Clinical Triage Hub is the central point of contact for community health referrals in Leeds. Our experienced clinical staff assess information from GPs, hospitals, social care teams, and other professionals to ensure each person is directed to the service that best meets their needs.
Home ward frailty
The home ward provides hospital-level support for people who are unwell but can safely receive treatment in the comfort of their own home. Our multidisciplinary team works together to offer personalised care plans.
Response team
The Response team provides rapid support for adults at risk of being admitted to hospital due to a sudden change in health.
