This service sits within patient flow and has one clinical and operational layer. The service is split across 3 hubs north, south and west of the city.
The Neighbourhood Therapy team takes internal routine referrals which includes GPs and AR received referrals from the acute trust and community care beds (step-down).
The therapists support patients who are considered as needing an urgent response, where there has been a high priority need identified or routine therapy appointments. The team consists of occupational therapists, physiotherapists and therapy assistant practitioners.
The service operates between 8am to 6pm, 375 days per year.

Patient must have a Leeds GP
Patient must be over 16yrs of age
