Neighbourhood response

We’re a team here to help you stay well and independent in your own home. Our team includes a number of health professionals who work together to give you the care you need, when you need it.

We work closely with your GP, hospital teams, the Yorkshire Ambulance Service, and other community services to make sure you get the right support quickly and safely.

How we can help

• Avoiding hospital visits: If your health suddenly changes or you feel unwell, we can come to your home to provide short-term care and support, helping you stay at home safely whenever possible.
• Helping you settle home from hospital: After leaving hospital, we can support you at home, check your needs, and make sure you have everything to recover well.
• Supporting those who need extra help: We focus on people with multiple long-term health conditions, complex needs, or who may find it harder to get care, making sure everyone has fair access to high-quality support.
