What do we offer?
We currently offer different options of activity for you to choose from. Read the list below and discuss the option with your Cardiac Specialist Nurse. Alternatively, you can contact the service if you would like more information on any of the following options.
Home Exercise Programme (HEP)
The Home Exercise Programme option is an easy way to keep active in the comfort of your own home. A member of our team will contact you to arrange a 1:1 home assessment. They will then create a personalised exercise programme specifically for you. They will assess your cardiac condition, your ability and your current health and fitness level.
What’s included:
• 1:1 assessment in your own home
• ongoing support for the duration of the HEP plan
• a variety of exercises for all abilities
• easy to follow exercise cards to ensure safe and correct technique
• a CAP diary to set goals and targets and to record your progress
• post assessment in your own home to measure your progress.
MyHeart (Digital Resource)
This online programme is a user-friendly, digital resource that can be accessed from your smartphone, tablet or laptop. MyHeart gives you 24/7 access to a wide range of tools to help you self-manage your cardiac condition.

• cardiac rehab exercise programme
• educational videos and reading
• mindfulness and anxiety coping strategies
• smoking cessation support
• diet and lifestyle advice
• goal setting

Active Leeds (Leeds City Council – Heart and Lung Programme)
This programme offers 12 weeks of activity, supported by a specialist Health and Wellbeing coach. The programme is delivered at various local community centres and Active Leeds venues across Leeds.
What’s included:
• 12 weeks FREE membership to Active Leeds leisure centres including use of the gyms, pools and classes
• ‘Bring-a-Buddy’ discount if you would like to bring someone with you to your activities
• a pre-assessment to meet a Health and Wellbeing coach who will help you plan your activities
• check in contact with your coach to keep you motivated
• a post-assessment to review your progress and discuss how you can keep active long term
• after your post assessment, you will be eligible for a discounted Active Leeds membership.
Activities delivered by our specialist coaches include:
• Gym Clinics – supervised drop-in gym sessions
• Aqua Mobility
• ‘Keep Moving’ Circuits
• Strength and Balance – this 20 week course is aimed at adults aged 60+ who are at risk of falls.
Walking groups:
We are pleased to be able to offer the opportunity to join one of our walking groups as part of your exercise programme. These groups allow you to be outside in the fresh air, socialising with others while exercising (walking). Please ask a member of our team for further details of a walking group near you.
