Keeping active after a cardiac event: Your options

Cardiac Activity Programme logo

We currently offer a range of activity options to support you in becoming more physically active in a way that suits your needs and lifestyle.

Please take a moment to read through the options below. You can discuss these with your Cardiac team, who will help you decide which programme is most appropriate for you. If you would like more information, you are also welcome to contact our service directly.

Active Leeds: Leeds City Council Exercise Programme

This is a 12-week physical activity programme designed to support your recovery and help you build confidence in being active. It is delivered by specialist health and wellbeing coaches at various community centres and Active Leeds leisure venues across the city.

What’s included:

  • Free 12-week membership to Active Leeds leisure centres, giving you access to:
    • Gym facilities
    • Swimming pools
    • Group exercise classes
  • Pre-assessment appointment with a Health and Wellbeing Coach to discuss your goals and create a personalised activity plan.
  • Ongoing support from your coach, including regular check-ins to help keep you motivated and on track.
  • Post-assessment review at the end of the programme to celebrate your progress and explore options for staying active long-term.
  • Discounted membership available after completing the programme to help you continue your activity journey.
  • ‘Bring-a-Buddy’ discount if you’d like to attend sessions with a friend or family member.

Specialist-led activities available

These sessions are delivered by trained coaches and are tailored to support your recovery and improve your physical wellbeing:

  • Gym clinics: Supervised drop-in sessions to help you feel confident using gym equipment safely.
  • Aqua mobility: Gentle water-based exercises to improve flexibility and strength with reduced impact on joints.
  • ‘Keep moving’ circuits: Light circuit-based activities designed to improve stamina and overall fitness.
  • Strength and balance course: A 20-week programme aimed at adults aged 60+ who may be at risk of falls. This course helps improve stability, coordination, and confidence in movement.

Home Exercise Programme (HEP)

Supporting your recovery from the comfort of home

The Home Exercise Programme is a flexible and accessible way to stay active, especially if you prefer to exercise in your own home or are unable to attend group sessions. This option is designed to support your heart health and overall wellbeing through a personalised plan tailored to your needs.

What to expect

Initial 1:1 assessment

A member of our team will contact you to arrange a one-to-one assessment. During this session, they will:

  • review your cardiac condition
  • assess your current health, fitness level, and mobility
  • discuss your goals and preferences for physical activity

Personalised exercise plan

Based on your assessment, you will receive a customised exercise programme that is safe, achievable, and suited to your ability. The plan will include a variety of exercises to help improve your strength, stamina, and confidence.

Easy-to-follow exercise cards

You’ll be provided with clear, illustrated exercise cards that guide you through each movement step-by-step. These are designed to help you perform exercises safely and correctly.

Ongoing support

Throughout the duration of your programme, you’ll receive regular check-ins from our team to:

  • monitor your progress
  • answer any questions
  • adjust your plan if needed
  • keep you motivated and on track.

Cardiac Activity Plan (CAP) diary

You’ll be given a CAP diary to help you:

  • set realistic goals and targets
  • record your daily activity
  • track your progress over time

Post-programme assessment

At the end of your programme, we’ll carry out a follow-up assessment to:

  • review your progress
  • celebrate your achievements
  • discuss next steps to help you stay active long-term

This programme is ideal if you prefer to exercise independently, need a more flexible approach, or want to build confidence before joining group activities. The Cardiac team can help you decide if this option is right for you.
