Clinical Triage Hub
The Clinical Triage Hub is the first point of contact for people referred to the neighbourhood nursing and therapy teams within Leeds Community Healthcare. Our aim is to make sure every person gets the right care, from the right professional, at the right time.
When a referral is received, whether from a GP, hospital, or another health or care professional, our experienced triage clinicians review the information to decide the best next step for the patient.
How the Clinical Triage Hub helps
- Fast and appropriate care: We assess each referral quickly so that people can access the right service without unnecessary delays.
- Expert advice and decision-making: Our team uses their clinical expertise to decide whether someone needs an urgent response, a community team visit, or another type of care.
- Joined-up working: We work closely with GPs, hospitals, and community services across Leeds to ensure care is well-coordinated and consistent.
- Supporting people to stay well at home: Wherever possible, we aim to help people get the support they need in their own homes and communities, avoiding unnecessary hospital visits.
The Clinical Triage Hub streamlines processes to ensure smooth and coordinated care, ensuring people in Leeds receive safe, timely, and person-centred care that meets their individual needs.
The Clinical Triage Hub is available between the hours of 7am to 6pm seven days a week.
