New partnership to support recovery across Leeds

From 1 July, people across Leeds have benefited from the Leeds Short-Term Community Beds Partnership, the new name for what was previously known as Community Care Beds or Recovery Hubs. The service has brought health and social care staff together as one team to support people across Leeds with their recovery, rehabilitation and independence.
The new contract builds on the work already taking place through Community Care Beds, also known as Recovery Hubs. Delivered through a partnership between Leeds Community Healthcare NHS Trust, Leeds City Council and Leeds GP Confederation, the service continues to provide short-term support for people who need care and rehabilitation, with the aim of helping people return home and live independently.
The partnership approach sees staff from across health and social care working more closely together, helping to deliver seamless support for patients and improving care for people and families across Leeds.
Sam Prince, Executive Director of Operations at Leeds Community Healthcare NHS Trust, said:

Linda Mac Donnell, Deputy Director, Social Work and Social Care at Leeds City Council, said:

By bringing social care, community health and primary care closer together, we can provide more coordinated support that focuses on what matters to each person and their family."
The Leeds Short-Term Community Beds Partnership supports people to recover closer to home, helping more people regain their independence, with health and social care professionals working in a much more joined-up way across Leeds.
