The Community Stroke Rehabilitation team

We are the Community Stroke Rehabilitation team and we deliver stroke specialist rehabilitation in the community for up to twelve weeks.

About the Community Stroke Rehabilitation team

Who do we see?

People who…

  • Have had a stroke
  • Are medically stable
  • Are over 18 years old
  • Have a Leeds GP
  • Are referred from hospital
  • Can be safely cared for at home
  • Have rehabilitation goals that can be met in 12 weeks

How can we help?

  • By working with you on goals that you have identified are important to you
  • Supporting the transfer of care from hospital to home
  • Aiming to improve your quality of life
  • Aiming to increase your independence
  • Providing advice for you and your carer
  • Working together with other services

Who we are

The team includes:

Group of 6 diverse people

  • Physiotherapists
  • Occupational Therapists
  • Speech and Language Therapists
  • Dietitians
  • Nurses
  • Clinical Psychologist
  • Therapy Assistant Practitioners
  • Rehabilitation Assistants

What we do

We offer three different pathways including Self-management, Rehabilitation and Living Well. The pathway that best suits your needs will be agreed whilst you’re in hospital.

We have a variety of ways of delivering care. These include:

  • Telehealth (groups, phone, video calls and apps)
  • Clinic appointments
  • Home visits

Once your episode of care is complete, we will write to you, your GP and others involved in your ongoing care.

If you require further support, we can refer or signpost you to other services that can help.
