What we discussed today
You have a musculoskeletal (MSK) problem that is affecting your usual activities or work. You’ve been given some advice and exercises, which you should continue.
We’re reassured that this isn’t a serious problem, but we understand it’s impacting your daily life and may be causing significant discomfort or concern.
What happens next?
You can now refer yourself to see a specialist physiotherapist at your local health centre, there’s no need to return to the GP practice for this.
When you click through to our SystmConnect page on a mobile device please make sure you scroll down and press the “Self-referral” button.

If you don’t have “SystmOnline” or “NHS login” details this doesn’t matter.
If you press either button and can’t login just press or swipe “back” and proceed to click on the “Self-referral” option.
If you have any technical difficulties using the online form, please contact the MSK service on 0113 843 1909 we can only support you to complete the form.
