What we discussed today
You have a musculoskeletal (MSK) problem that is affecting your usual activities or work. You’ve been given some advice and exercises, which you should continue.
We’re reassured that this isn’t a serious problem, but we understand it’s impacting your daily life and may be causing significant discomfort or concern.
What happens next?
You can now refer yourself to see a specialist physiotherapist at your local health centre, there’s no need to return to the GP practice for this.
Complete our online referral form.
When you visit the above click on the “Self-referral” option.
If you have any difficulties using the online service, please contact your GP practice and ask them to complete a professional referral. Please do not contact the Leeds MSK service, as they will not be able to help with technical issues or queries about the referral process.
While you are waiting, if you get any new symptoms or things get worse, contact the GP practice as soon as possible.
