SR1 form (previously known as DS1500)
The SR1 form is completed by a doctor or other healthcare professional to confirm a person is terminally ill to expedite claims for benefits, such as, personal independence payment (PIP), disability living allowance (DLA), and attendance allowance.
Attendance allowance (AA)
Attendance Allowance helps with extra costs if you have a disability or health condition severe enough that you need someone to help look after you. There’s a different way to claim if you’re nearing the end of life (for example, because you have a life-limiting illness).

Personal independence payment (PIP)
Personal independence payment (PIP) is a benefit for people who are under State Pension age and need help with daily activities or getting around because of a long-term illness or disability. It isn’t based on your National Insurance contributions and it isn’t means-tested, so your savings don’t matter.
Disability living allowance (DLA)for adults
Disability Living Allowance (DLA) is money for people who have extra care needs or mobility needs (difficulty getting around) as a result of a disability.

Enhance programme (Leeds Older People’s Forum) is a partnership between 4 third sector delivery partners and LCH services, providing person-centred, holistic support for people at home receiving community-based healthcare. Enhance support is available for people over 50 living in the Armley Moor, Beeston, Chapeltown, Middleton and Seacroft Neighbourhood team areas.

Telecare help alarms and safety in the home.
A telecare alarm system can help you live safely and independently at home.
Leeds Tele Care can provide 24-hour monitoring of alarms and sensors, which sends an alert to the response centre, where staff can get you the appropriate help.
