The Community Neurological Discharge team (NDT) is a small team of specialist occupational therapists and a therapy assistant practitioner. We are part of the wider community neurological rehabilitation service based at St Mary’s Hospital.
We accept referrals directly from Leeds Teaching Hospital’s therapists and aim to support you as you transition home from hospital.
Referrals
We accept referrals for people who have suffered an acquired brain injury, undergone neurosurgery or have experienced a hypoxic brain injury (such as following a cardiac arrest). You must have a Leeds GP, be over the age of 16 (and have left high school) to be accepted for the service.
We will work closely with your hospital therapists to plan a safe and successful discharge, providing rehabilitation, advice and guidance in the early days as you adjust to being back home.
We sometimes will meet you on the ward before you go home and will make contact with you within 48 working hours of your discharge.
We will work with you and your close family, carers or relatives for up to 12 weeks once you are home, working towards goals that are meaningful to you. We will also provide education to you and your family or support network to help you come to terms with your newly acquired brain injury or as you recover following surgery.
The service is available Monday to Friday between the hours of 8am and 4pm (please note we do not work on bank holidays).
We have close links with other local services and will support you to access them if needed. At the end of our input we can also refer to other teams so that you can work towards your longer term goals.
