Following discharge from hospital you will be contacted by the service to advise when you’re likely to be seen. If you have any concerns or questions whilst waiting you can contact us on the number below. We will then complete an initial assessment which identifies what your goals are, and which therapists need to be involved in your rehabilitation.
How can we help?
How can we help?
We are a goal focused service providing stroke rehabilitation aiming to improve your independence and quality of life. This includes advice and therapy for you, and advice and support for your families and carers. We also work alongside other services who may be involved in your care and can support your transfer from hospital to home.
Therapy is often provided at home or within the community. We can also offer virtual and group sessions. Therapists may also provide rehabilitation exercises for you to complete between visits.
The team will meet regularly to discuss your goals and how best to support you.
Once your episode of care is complete, we will write to you, your GP and others involved in your ongoing care. If you require further support, we can refer or signpost you to other services that can help.
