The Leeds active recovery service support people at home to restore ability, confidence and independence.

Who are the active recovery service?
We are a local community team made up of health and social care professionals.
Active recovery is a short-term service which supports people to regain their ability and confidence to do everyday activities.
When your period of support comes to an end with active recovery and if you need some ongoing support, there may be a charge. This will be discussed with you.

Our team aim to make sure that you have the right support, at the right time, in the right place and by the right person.
We will:
- introduce ourselves and display our ID badge
- treat you as an individual
- be respectful to you and those around you
- value your privacy and dignity
- be polite and helpful
- keep your personal information secure.
What we offer
What we offer
A member of staff will visit you to complete a detailed assessment to identify your needs and what matters to you.
We will agree a plan with you to support your recovery. This may involve exercises, mobility, stairs practice, advice, provision and use of equipment and support to learn or relearn how to prepare meals and wash and dress yourself.
We encourage people to do as much as they can for themselves

If you need to cancel a visit, please let us know.
We will need to wash our hands before we support you. Please provide access to a sink to enable us to do this.
We need to work in surroundings that are smoke free, safe, and free from abuse and discrimination.
Please ensure any pets are safely secured.
Please work with us to create a safe environment for all involved in your rehabilitation.

Consent and confidentiality
We will ask for your consent to work with you and to share your information with other health and social care professionals when needed.
You can choose not to give us permission to share your records. This will not stop you from receiving care but may limit how we are able to support you.
There may be times where we need to share your information even if you have told us that you do not want us to do this. If necessary, we will discuss this with you.
The information we collect is confidential and will be kept securely on a single care record.

If you have a compliment, complaint, concern, please let us know by speaking to a member of our team. We learn from your feedback and use the information to improve our services.
