The Leeds active recovery service support people at home to restore ability, confidence and independence.
Who are we?

Leeds Active Recovery is a short-term, community-based rehabilitation service that supports people in their homes to regain independence and confidence after a hospital stay or illness.
We are a local community team made up of Leeds City Council reablement and Leeds Community Healthcare therapy professionals.
What we offer
- Following a detailed assessment, we will agree a plan with you to support your recovery. This may involve exercises, mobility support and advice, and provision and use of equipment. You may need support with meal preparation, washing, dressing and medication.
- We will work with you to jointly agree your goals based on your current needs.
- We will encourage you to do as much you can for yourself to become as independent as possible.
- You will be involved in regular reviews of your progress.

- If you need to cancel a visit for reablement or therapy, please contact us on the numbers on the back page.
- We will need to wash our hands before we support you. Please provide access to a sink to enable us to do this.
- We need to work in surroundings that are smoke free, safe and respectful.
- Please ensure any pets are safely secured.

- We will ask for your consent to work with you and to share your information with other health and social care professionals when needed.
- You can choose not to give us permission to share your records. This will not stop you from receiving care but may limit how we are able to support you.
- There may be times where we need to share your information even if you have told us that you do not want us to do this. If necessary, we will discuss this with you.
- The information we collect is confidential and will be kept securely on a single care record.

When your care with the Active Recovery team finishes, if you need extra help with daily activities, there may be a charge (this doesn’t apply to therapy).
Don’t worry, we’ll talk this through with you and make sure you understand everything.
